Registration Information Required
Required Documentation
Parent/Guardian
Copy of Picture ID (License or State ID)
Social Security Card
Required Documentation for
All Children 12yrs old and under
Copy of all participating children’s Medicaid Card
and
Copy of all participating children’s Birth Certificates
Required Financial Information
Copy of your income/budget statement (FIA/SSI/Other)
Copy of your last pay stub (within last two week preferred)
Copy of your bridge card
